I confirm that to the best of my knowledge and belief, my pet is in good health and free from any injury or physical disability. I agree to and advise that my pet has been vaccinated and I understand that If I do not keep my pet vaccinated the insurer will not pay any claims resulting from any illness that it should have been vaccinated against. (Dogs: distemper, hepatitis, rabies, leptospirosis and parvovirus. Cats: feline infectious enterirtis, feline leukaemia and cat flu.)

I confirm that my dog is not any dog that should be registered under the Dangerous Dogs Act 1991 and the Dangerous Dogs (Northern Ireland) Order 1991 or subsequent amendments, or Spanish Law 50/1999 of 23rd December relating to the legal status of potential dangerous animals ownership and related subordinate legislation as a potential dangerous animal. All types of Pit Bull Terrier, all types of Staffordshire Bull Terrier, Rottweiler, Akita Inu, Japanese Tosa, Sharpei, American Pit Bull Terrier, Dogo Argentino and Fila Brazileiro or dog crossed with these breeds, wolves or wolf hybrid.
